About the job
Position title: Neurorecovery and Rehabilitation Program Patient Care Navigator.
Schedule: Per diem (5-20/hrs weeks).
Primary Location: Remote.
Reports to: Patient Care Navigator Lead.
Work in collaboration with: Program Director, clinical care providers (occupational therapists, speech & language pathologists, physical therapists, mental health workers, etc.).
Description
We are seeking a dedicated, person-centered, and motivated individual to join our team as a patient navigator! This person will guide stroke patients in every aspect of their recovery care journey and will be responsible for coordinating the comprehensive care plan. The Patient Navigator will also ensure that the patient adheres to the established plan of care while adjusting the model as needed.
Responsibilities
- Establishes close relationships with and serves as primary point of contact and advocate for clients (patients and their caregivers).
- Responsible for logistics around scheduling for client and Care Team members (Care Coordinators, primary care physicians and other health care providers).
- Client intake and screening.
- Provide program introduction and orientation with client.
- Leads and facilitates care team (i.e., treating clinicians) meetings to ensure efficient and effective discussions of necessary client cases and personalized program development.
- Track client information, schedules, files, and forms in a confidential manner.
- Identify resources for clients to overcome barriers to care, such as technology troubleshooting, caregiver engagement, etc.
- Maintain documentation of all client encounters and complete reporting requirements according to organization standards.
- Maintain strict confidentiality in accordance with agency policies.
- Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.
Qualifications

- Passionate, trustworthy, and empathetic when working with clients.
- Ability to build relationships with different types of people, including clients, organization members, and health care providers.
- Good communication and interpersonal skills and ability to speak concisely to clients and Care Team members.
- Organized with confidential client material and appointment tracking.
- Flexible and adaptable in response to changing client and health care providers’ needs.
- Interest in working with persons with stroke.
Education and experience:
- Bachelor’s degree in nursing, social work, clinical counseling, public health, psychology, biology, or other related field.
- Master’s degree in nursing, social work, case management, or related field preferred but not required.
- Strong understanding of the complex presentations of the target population (i.e., stroke)
- Computer literacy desirable.
- Exposure to issues of rehabilitation.
